    About this opportunity:The British Red Cross Assisted Discharge Service supports Fife patients during the transition from hospital to home
     •  We will initially meet with the person using the service the day before to allow a better understanding to meet needs required and to make the service user more comfortable and confident leaving the hospital
     •  On the day of discharge the service user will be collected/dropped off via hospital transport from agreed hospital and supported to settle back in their own home
     •  Support time can vary depending on needs of service user
     •  A follow up call 24/48 hours later again depending on the needs of the service user

    Training provided:A 3 day course of Foundation Training Programme.
