    Post: Hospital Liaison Case Worker 
    Salary: £29,385.72 per annum
    Location: Travel effectively between service locations throughout Fife and the team base at Victoria Hospital, Kirkcaldy

     

    The main duties and responsibilities of the role include: 

    • Contribute to ADAPT’s (REACH TEAM) component of the Seek, Keep, Treat Multi-Disciplinary Team (MDT) provision within Hospitals and community locations across Fife, by providing case management, therapeutic psycho-social support and crisis/relapse intervention for patients/clients following medical treatment for alcohol and drug related conditions, accidents and overdoses.
    • Act as a ‘Named Person’ whom patients/clients can rely on for contact and support during their journeys through services by establishing ongoing therapeutic relationships that normally begin with contact in hospital and proceed into the community.
    • Provide Case Management for patients/clients ensuring linkage and communication between patients/clients and services. Liaise with Care Coordinators, Addictions Nurses, We Are With You Support Workers and specialist Pharmacist within the MDT and with other professionals within Health & Social Care, the Statutory and Third Sectors.
    • Provide Fife ADP Triage Assessment and Referral via FORT to other addiction services within statutory and third sectors. Ensure Triage Assessments are forwarded timeously to enable 90%+ of all Service Users to enter treatment within 21 days of referral date.
    • Provide Client Support with crisis intervention for clients who are living chaotically and/or are at risk of mental and physical trauma, overdose or self-harm and suicide.
    • Provide structured Motivational Interviewing and Psycho-Social therapeutic support for patients/clients who are not willing or ready to attend treatment and rehabilitation services following hospital.
    • Maintain records and provide information for service reporting in accordance with organisational policy and practice.
    • Participate in annual appraisal and regular supervision and be responsible for personal Continuous Professional Development.
    • Carry out any other duties that may reasonably be requested.

    Essential Criteria 

    • A professional qualification at Diploma/Degree or equivalent 
    • Work with patients/clients with complex health and social care needs.
    • Therapeutic and/or support work with substance misuse patients/clients
    • Experience of basic care for substance using patients/clients
    • Excellent ability to work with substance using patients/clients, their families and friends in a supportive, empathic and non-judgement manner
    • Excellent networking and communication skills, including telephone, social media, written and verbal
    • Ability to organise and prioritise work, manage time and case workload
    • Ability to work within a Multi-Disciplinary Team, take responsibility for agreed areas of work and work on own initiative
    • Basic IT skills for recording patient/client records  
    • Ability to work out of hours evening shifts on rotation as required
    • Ability to travel effectively between service locations throughout Fife and the team base at Victoria Hospital, Kirkcaldy
