    Post: Social Prescribing Practitioner
    Hours: 35 per week
    Salary: £26,800 per annum - (12-month fixed term)

    FDAMH are seeking to recruit a skilled and experienced Social Prescribing Practitioner to join their team.

    With the knowledge and understanding of the impact for a person experiencing reduced psychological wellbeing, you will work with a wide range of people providing support using a range of holistic, therapeutic techniques.

    The post requires someone with experience of conducting assessments, creating and implementing support plans, while monitoring and supporting necessary changes quickly.
