Salary: £25,500.00 per annum
Falkirk’s Mental Health Association are seeking to recruit a skilled and experienced Family Social Prescribing Practitioner to join their team in the Carers and Family Support Service. With the knowledge and understanding of the impact of living within a family where someone is experiencing reduced psychological wellbeing, you will work with a wide range of people providing support using a range of therapeutic interventions. The post requires someone with experience of conducting assessments, creating and implementing support plans, while monitoring and amending necessary changes quickly.
